Minimum age for participants is 11 yrs.
This form must be completed by the parent/guardian of anyone under 16 yrs
By signing this form you give consent for your child to take part in airsoft skirmishing events at Ace Combat, you and your child have read and agree to our site rules/terms & conditions.
I declare that my child is fit and in good health and has no known medical conditions that would incapacitate him/her during the day

Equipment

I agree that all the equipment hired to your child will be returned at the end of the event. Any damaged or missing equipment may incur a charge.

Parent/Guardian’s signature of consent

You will be indicating your consent to receiving email marketing messages from us unless you have indicated an objection to receiving such messages by leaving the box empty.
